Acceptance also depends on things only a person can judge, such as focus, lighting, shadows and expression, and the issuing office decides. Check its current rules before you apply.
Download the print sheet. It is a 4×6 in (10×15 cm) photo at 300 DPI with as many copies as fit, for example six US 2×2 in photos or eight 35×45 mm photos. Order it as a standard 4×6 print at a pharmacy, supermarket kiosk or online photo lab, then cut along the grey lines.

It depends on the document, and it is set when you choose one: white or off-white for the US, China and India, cream or light grey for UK printed photos, light grey preferred in Germany, and light blue or light grey for France, where white is not allowed. You can change it.

Safest is no. US passport and visa photos must be without glasses, and the UK asks you not to wear them unless you have to. Some countries, such as Canada, allow them if your eyes are clearly visible with no glare or tinted lenses.
